Perry Middle and High Schools now have a School Wellness Program! This means that we have a nurse and therapist that can see students while they are at school. In order to be seen by the nurse or therapist, your student has to have a signed consent form. You should have received a postcard in the mail with a QR code to sign the consent form, if you did not, here is the link to the consent form. Copies of the consent form are also available in the Middle and High School main offices.
